Regardless of what scope of obstetrics care you provide, a good OB wheel app is a must.

With so many different OB apps available, which one should we use? My go-to OB wheel has been Dr. Evan’s app called Perfect OB Tracker. It combines an easy-to-use, accurate OB wheel with the option for patient tracking. This function automatically updates EGA, etc. every time you open the app and click on a particular patient.

Not everyone wants that functionality due to patient privacy concerns. What are other options for a simple OB wheel app? Once again, Joshua Steinberg, MD, not an app, but a family physician who develops great apps for primary care, comes to the rescue. In 2017 he was on fire with many new apps including Contraception, Health Maintenance Visit Checklist and updates to Pneumonia Guide and ABG Eval. Plus, many of his apps are now available for Android. Over the past 24 months, he has released apps that I have reviewed here on iMedicalApps, including PneumoVaccines, Depo Calendar, OB Wheels, Gout DiagnosisHealth Maintenance Visits, and Step-by-Step Febrile Infant. Other useful point-of-care apps by Dr. Steinberg are PreopEval14, PFT Eval,EFM Guide, PE & DVT Dx Tool, and my favorite, Pneumonia Guide. His most recent app is a redo of the venerable OB Wheel, an excellent medical app designed to assist providers in dating pregnancies and adjusting for sonographic data. His original app was good, but we felt the interface was too complicated and not responsive enough. He has redesigned the app to make it easier to use while reflecting the most current ACOG dating guidelines. The app consists of three main sections: a calculator based on Last Menstrual Period (LMP) or Estimated Date of Delivery (EDD); a “virtual cardboard” old-school OB wheel that you can adjust with your fingers; and a reconciliation guide that presents the most current ACOG Bulletin on dating pregnancies.

Clinical Scenario

You are seeing your new continuity obstetric patient today. The pregnancy appears uncomplicated based on prenatal labs and maternal history, but the patient is not entirely sure of her LMP. Your colleague performed the first-trimester ultrasound a week ago that shows a difference of 10 days between the ultrasound and LMP dates for the patient’s EDD. Which do you use? Although it lacks patient tracking capabilities, it is still a great first choice for a basic OB wheel app. It now also includes the latest ACOG dating guidelines.
