Stanford Letter Project
Having an end of life and goals of care discussion can be incredibly challenging but the importance of those conversations is hard to overstate. The Stanford Letter Project, an interesting effort from Stanford, is trying to empower patients to initiate and drive those conversations.
The app walks users through the process of writing a letter to their physician and care team that begins the discussion. Available in several different languages, users answer a series of questions that help describe their goals and values. It also helps them provide guidance on end of life decision making.
