Warm Health, a healthcare communications company, has released an iOS app for health plan members to communicate with care managers using a secure, cloud based communication channel.

Care managers can contact members in need of help using the chat feature, and members can respond at their convenience.

They can also keep a log of their mood, weight and various other data points.

The feature works with any of Warm Health’s interactive clinical programs, including it’s Maternal Health and Diabetes programs.

CEO Dan Frier points out that “the Warm Health App is the next step in the evolution of our member-focused, interactive communications platform.” In addition to the iOS app, this platform currently includes automated phone calls, text messaging services, email and web-based dashboards.

Warm Health was established in 2005 in Minneapolis, Minnesota by “healthcare and technology professionals to create a better way for health plans to communicate with their members.”

The app is currently available in the iOS App Store, and is expected to be available on the Android platform in June, 2012.
